FY 2011 Financial and Programmatic Forms

The Grants Administration Division provides required forms for organizations awarded grant contracts. If your organization has a grant contract with the OAG, these forms were personalized and emailed to your organization. Below are PDF sample documents, instructions, additional forms, and contact information if you have questions or need assistance.

Combined Reimbursement Forms (Due Monthly)

If your organization has a grant contract with the OAG, your personalized Combined Reimbursement forms were emailed to your organization.

Combined Reimbursement forms must be completed, signed, and submitted to:

Performance Report Forms (Due Quarterly)

If your organization has a OVAG, VCLG, or SAPCS-Federal grant contract with the OAG, your personalized Performance Report forms were emailed to your organization.

Performance Report forms must be completed and emailed to: OAG-Grants@texasattorneygeneral.gov.

If you are new to your organization and do not know the name of your grant manager, please email us at OAG-Grants@texasattorneygeneral.gov with your name, title, organization name, phone number and OAG grant contract number, if known.

Forms for Texas VINE Counties

The Crime Victim Services Division's Grant and Contract Management Section provides the forms needed for VINE Counties. If your county has been awarded a grant/contract, you can download the forms to your computer. You may then fill in data and information on your computer, print, sign, and send the form to:
